Measuring Success for the Resource Center for Community Health Services

The establishment of a resource center focused on providing community health services aimed at improving health outcomes for Black populations comes with clear expectations around required outcomes and performance metrics. This initiative is designed to specifically address healthcare disparities through preventive care and wellness education while ensuring that funding supports systemic change rather than just episodic service delivery. Unlike more generalized health funding, this initiative does not support clinical services that do not incorporate a community health education component.

Required outcomes will be closely monitored with specific benchmarks in place to evaluate the effectiveness of workshops offered, participation rates, and health improvements in the population served. Metrics such as the percentage decrease in chronic disease prevalence among participants and increases in engagement in preventive health screenings will be prioritized. These benchmarks will provide critical data that can drive future funding decisions and program adjustments.

Key performance indicators (KPIs) necessary for tracking progress will include: 1) the number of community members accessing preventive care services, 2) participant retention across workshops, 3) improved health literacy scores, and 4) documented health outcomes such as reduced emergency room visits due to preventable conditions. Each of these metrics will provide essential insights into the program's reach and effectiveness.

Additionally, strict evaluation and reporting requirements will be instituted.
